Get prospectus/admission form from the Admission office of CIMS campus  

   Fill the form in capital words (all column should be filled).

· Attach Complete attested documents as mentioned below .

Documents to be attached For Admission

• Four (6) color photocopies of SSC Result Card/ Certificate issued by the board.
• Six (6) Coloured Photographs with sky blue background .
• Four (4) color Photocopy of Students’ CNIC /Form B.
